If you have several email accounts setup in Outlook express it might pay to periodically check each one separately instead of just clicking the Send/Receive button.
Try Clicking the small v Arrow next to the Send/Receive on the Right hand side and check each email account separately.
You may find this downloads several emails for that email account, many of the emails could be days, weeks or even months old.
This is not a server issue its an Abnormality with Outlook express which I noticed with email accounts in my list numbered 13 and above.
( no i am not superstious but makes you wonder sometimes ).
